CTL-T-23-02 APPLICATION FOR AN AMENDED CPCN ) AND (2) PETITION TO DEMONSTRATE ) SECOND AMENDED SATISFACTION OF REQUIREMENTS TO ) CERTIFICATE NO. 348 HOLD CPCN NO. 348 AND DESIGNATION ) AS AN ETC ) IT IS HEREBY CERTIFIED that this amended Certificate of Public Convenience and Necessity, Second Amended Certificate No. 348, is issued to CTC Telecom, Inc. ("CTC") to remove CTC's right to provide voice and broadband services over fiber to the home facilities in Ada County,Idaho. CTC remains authorized to provide fixed wireless services within Ada County, Idaho. The Commission initially authorized CTC to provide Title 62 telecommunications services, including basic local exchange services, as a competitive local exchange carrier within parts of the state of Idaho. The Commission approved CTC's Certificate by Order No. 27673 issued August 10, 1998, which limited the areas in which CTC was authorized to provide telecommunications services, as depicted in the map attached to the original Certificate as Exhibit A and as fully described therein. The Commission further approved an amendment to CTC's Certificate by Order No. 28059 issued June 8, 1999 to allow CTC to provide competitive local exchange service as a competitive local carrier throughout the U S West Communications, Inc.'s southern Idaho service territory. This Second Amended Certificate is predicated upon and is issued pursuant to the findings of the Commission in Order No. 3 627 1. 1H SECOND AMENDED CERTIFICATE NO. 348 1 DATED at Boise, Idaho this 301h day of October 2024.
