I've just sent you a copy of the letter sent to me by Zlply.
Please add this to my formal complaint.
I put in a work order on 10/18/24 because my line has so much static that I can not make a call because
it dials different numbers not the one I dialed. I can not even dail 911.
Richard Jayo called me yesterday 10/19/24 and told me that there was nothing he could do to fix the
line, because it is on the ground. He said if they come in they will reset the equipment, but will not work
on the line.
Please add this to my formal complaint as they are braking rule 500.02 again.

Subject: 6008625 CIAC EKCYIDXA REPLACE DAMAGED CABLE FROM C.O. TO GOLDEN
This is in response to your request for Ziply Fiber to perform the following work: Ziply will replace the existing copper
cable running from the Elk city central office to Golden ID.That is approximately 17 miles of placement through rough
terrain with very poor access to the existing pole lead. Many of the said poles will need to be replaced as well. Placing
a new copper cable will solve the ongoing service issues.
We have estimated that your cost of this work effort will be: $ 2,164,945.59
Less the Advance Engineering Design Payment: $ 0.00
Balance due to Ziply Fiber: $2,164,945.59
Once we have received your signed Letter of Intent (LOI)you will be emailed a Ziply Invoice for the agreed upon
amount. Please send payment to the address on the invoice. Your work will be scheduled with our construction
department once payment is received.
Upon job completion, you may be issued either: (1) a refund for any overpayment, or(2) an invoice, if the final actual
costs exceed the advance payments received.
If you agree to these terms, please sign below, and return the signed letter by email to jon.cortabarria1(cDziply.com
Ziply Fiber shall not be responsible to the extent its performance is delayed or prevented due to causes beyond its
control, including but not limited to acts of God or the public enemy, terrorism, civil commotion, embargo, acts of
government, any law, order, ordinance, regulation, or requirement of any government, fires, explosions, weather,
quarantine, strikes, labor disputes, lockouts, and other causes beyond the reasonable control of Ziply Fiber.
Please be advised that the price estimate quoted above is only valid for sixty(60) days from the date of this letter.
If this work request is cancelled after you have signed the agreement, you will be billed for any Engineering and
Construction cost incurred after the date of signature that may include the cost to place and/or remove facilities.
If we do not receive this signed agreement and your full advance payment within this sixty (60) day period, we will
assume that you do not want the work to be undertaken and the project will be cancelled.
