Inspection Summary
A construction inspecton was performed on Das-co, a contractor for Intermountain Gas Company
21 Dec 2021 (2.0) contruction)
Observed a Das-co crews installing 6" sleeve by pulling the sleeve back to tie in point off Avalon St in Kuna,
Idaho.While excavating the trench in preparation for pulling 4"main through the sleeve the contractor struck a
'/4"water line with shovel and caused a leak. Dasco supervisor upon seeing the leak, followed Idaho dig line laws
and contacted the owner of the water line and provided support to the city of Kuna to stop the leak. '/4"water
line was severely pitted and hence contractor is not being held liable for the leak. All procedures were followed.
