Inspection Summary
Audit consisted of inspecting field installed operating equipment for the Avista's Kellogg,Idaho inspection
unit.Records and procedures for all Idaho Avista units were inspected March 16-17,2021 virtually from
our office in Boise,Idaho.
June 21,2021: (3 Standard travel)
Traveled to Coeur d'Alene, ID to conduct inspection.
June 22-23 2021: (5 Standard) (1 OQ)
Met with Avista company representatives in the Kellogg, Idaho district office for in-brief and began field audit of
Kellogg installed equipment.
Regulator Stations 230,235,240,242,244,248,249,252,260270,273,274,275,276 and 278, were randomly selected
and inspected for relief operation, regulator lock-up and general condition of the station including pipe-to-soil
wrap and corrosion. Seven cathodic rectifiers,twenty valves, six carrier-to-casings and three patrols were
inspected for the Kellogg district with four defects noted(Reg Sta#235 and 260 worker failed lock-up and 270
monitor failed lock-up also Patrol at Terror Gulch-Expansion joint possible out of range deflection and pipe is
lifting off the rollers). IPUC Form 3 was conducted with Randy Bareither.
June 24,2021: (3 Standard)
Traveled back to Boise, ID. Wrote the inspection report, completed audit forms: PHMSA IA question set, IPUC
Forms 1, 3 and attached inspection documents.
