2021, CRM, Records, and Field Inspection Dominion Energy
September 9, 2021: (1 Standard Prep)
Prepared for the Dominion Energy records/CRM inspection through review of the PHMSA inspections Form 2
and 2020 inspection results. The company's pipeline system in Idaho consists of transmission and distribution
lines.
September 13, 2021: (.5 Standard Travel, .5 CRM)
Traveled from Boise, ID to Salt Lake City, UT and met with Dominion Energies compliance and engineering
personnel and Control room personnel at Dominion's Office/CRM to review records and CRM procedures.
September 14, 2021: (1.0 Standard)
Idaho Public Utilities Commission Inspector met with Dominion Energies compliance and engineering personnel
at Dominion's Offices in Logan, Utah to review records for the previous year 2020. The PHMSA Form 2
(Standard Inspection Report of a Gas Distribution Operator) and PHMSA Form 13 (D&A Inspection Form) were
used as a guides throughout the audit to ensure all areas were completed as they pertain to required records.
September 15, 2021: (.5 Standard/Travel, .5 OQ)
Met with Dominion Energy representatives in the Logan district office for in-brief and to begin field audit.
Regulator Stations FR0001, FC0001, FC0003, FC0004, FC0005, FC0006, and PS0001 were inspected for proper
operating parameters, regulator lock-up, relief device operation, cathodic protection pipe-to-soil readings, pipe
to soil interface wrap protection, station warning signs and right-of-way clearances.
Three casing-to-carrier isolation checks, one cathodic protection rectifier and nine emergency isolation valves
were checked for proper operation and found to be operating properly.
An IPUC Form 3 (exit interview) was performed following the inspection with Justin Berg(Pipeline Compliance),
Clark Jarrett(Measurement& Control (M&C)Tech), Kelly Facer(CP Tech),James Sharp (M&C Tech), Reid Hess
(Manager Gas Control) and Redd Ivins (CRM Shift Lead) of Dominion Energy.
Observations:
Reg Stations: FR0001, FC0001, FC0003, FC0004, FC0005, and FC0006 not plumbed for lock-up, but were able to
manipulate to ensure lock-up would occur.
