Inspection Summary
Audit consisted of inspecting field installed operating equipment for the Avista Coeur D'Alene,Idaho
inspection unit. Records and procedures for all Idaho Avista units were inspected March 16-17,2021
virtually from our office in Boise,Idaho.

IPUC inspectors Brooks, Perkins and Galli conducted a field audit of Avista's CDA district. Audit included:
Review of construction records,welding records, emergency manual, verification of Steve Finney's OQs, 7
regulator stations (212, 214, 4246, 4247, 2210, 220, 5001-1) 6 carrier/casings,4 rectifiers, 16 control valves and
no patrols. Traffic control was utilized for several control valves. Visited 6997 W Christine St in Rathdrum
Idaho which was the site of federal reportable incident 20210098-36039. Took photos and examined scene of
house explosion.
Three regulator stations (212, 4246, 4247) failed lockup due to dithiazine issues. All were fixed on-site by
replacing pilot stems or diaphragm. Failed regulator stations will be revisited in 2023. Reference photos saved in
CDA field audit folder.
