Inspection Summary
Audit consisted of inspecting field installed operating equipment for the Avista Kellogg, Idaho inspection
unit.Records and procedures for all Idaho Avista units were inspected March 16-17,2021 virtually from
our office in Boise,Idaho.
February 17, 2022: (0.5 Standard Prep)
Prepared for Kellogg Field Audit.
June 6,2022: (3 Standard Travel)
Traveled to Coeur D'Alene to conduct inspection
June 7, 2022: (2.5 Standard 0.5 OQ)
Met with Avista company representatives in the Kellogg district office for in-brief and began field audit of
Kellogg installed equipment. This field audit included: Review of the emergency operations manual,
construction and work order records, OQ verification for David McGregor,4 regulator stations (235, 253, 260,
270), 8 control valves, 8 carrier to casings and 3 patrols. No rectifiers were inspected.No discrepancies

Wrote the inspection report, completed audit forms: PI MSA IA question set, 1PUC Forms 1, 3 and attached
inspection documents.
