Inspection Summary
Audit consisted of inspecting field installed operating equipment for the Avista Lewiston, Idaho
inspection unit. Records and procedures for all Idaho Avista units were inspected March 16-17,2021
virtually from our office in Boise,Idaho.
February 14,2022: (0.5 Standard Prep)
Prepared for Lewiston field audit.
April 4,2022: (3 Standard travel)
Traveled to Lewiston, ID to conduct inspection.
April 5-6, 2022: (5.5 Standard 0.5 OQ)
Met with Avista company representatives in the Clarkston, Washington district office for in-brief and began
field audit of Lewiston installed equipment.
Regulator Stations 401,402,404,406,410,414,415,416 and 450 were randomly selected and inspected for relief
operation, regulator lock-up and general condition of the station including pipe-to-soil wrap and corrosion. Four
cathodic rectifiers, fourteen valves and three carrier-to-casings were inspected for the Lewiston district with one
defect noted(Reg Sta#414 worker failed lock-up). Two valves were not able to be turned because of traffic
access LEC71 and LEC 111.

Wrote the inspection report, completed audit forms: PHMSA IA question set, IPUC Forms 1, 3 and attached
inspection documents.
