Audit consisted of inspecting records from 2021 and 2022, and field installed operating equipment for the

IPUC Inspectors traveled to Logan,Utah to conduct a records and field audit with DEUWI.
October 11-12,2023: (3.0 Standard)
IPUC inspectors Galli and Brooks conducted a records audit on Dominion Energy at the Logan Office with Justin
Berg and Steve Bursett; Bruce Baylor was the SME for welding/welder qualifications. The PHMSA IA question
set for the records portion was used as a guide throughout the audit to ensure all areas were completed as they
pertain to required records. A complete list of attendees can be found in the IA inspection. All observed records
were found to be in compliance with applicable code sections.
October 12,2023: (1.0 Standard)
IPUC inspectors Galli and Brooks conducted a field audit of Dominion Energy's Preston District. Audit
included: Verification of David Platt and JR Sharp OQ Records. Inspection assets included: 7 regulator stations,
12 control valves and no patrols in district. No CP Tech available for audit, will audit CP assets in 2024. All of
the equipment inspected was in satisfactory operating condition and performed within regulatory compliance
requiremements.

Wrote the inspection report, completed audit forms: PHMSA lA question set including OQ protocol 9, IPUC
Forms 1, Post-Inspection Written Preliminary Findings and attached inspection documents.
