IPUC inspector Galli partially observed Das-Co a contractor for IGC install a 1/4" service line.
To address: 1988 S Cactus Lily Way
Observations included: trenching, line locating, tracer wire and butt fusion. Quizzed employees on
applicable AOCs of fusion procedures. During a construction inspection on Service Line# 047771-413
at 1988 S Cactus Lily Way in Meridian, ID, a contractor representing IGC dug a trench for a service line
installation using locate ticket 373562. That locate ticket is for 1948 S Cactus Lily Way, Meridian, ID.
Per the 811-call center. The correct locate ticket for 1988 S Cactus Lily Way, Meridian, ID was 332847
and expired on 9/12/23.
To address: 1955 S Cactus Lily Way
FOC John and Dasco Foreman Jordan Oakes were notified that the locate ticket for address 1955 S
Cactus Lily Way was expired. With the locate ticket 352884 having an expiration date of 9/28,
excavation needed to be started by 9/21, pursuant to Idaho Statute Section 55-2205.
"Unless otherwise agreed in writing by the parties, maintained markings shall be valid for
purposes of the notified excavation for a period of no longer than four (4) consecutive
weeks following the date of notification as long as it is reasonably apparent that site
conditions have not changed so substantially as to invalidate the markings. If excavation has
not commenced within three (3) weeks from the original notice to underground facility
owners through the one-number notification service, the excavator shall reinitiate notice in
accordance with this section.

Locate ticket 373562 expiring on 10/17 was incorrect for this address. Per the 811-call center the
most recent locate ticket for 1964 S Cactus Lily Way is 373561 and is set to expire on 10/17.
