To address: 232 Elmore Ave. Observations included: Welding, hot Tapping, coating removal, wrapping, safety equipment, trenching, bedding, backfilling, line-locating, fusing, stab fittings, tracer-wire, 1200 EFV install,pressure testing and purging. Quizzed employees on purging and cooling times after fusing. Not all procedures were observed in compliance with IGCs Operations and Maintenance Manual. Upon arriving at the job site, I observed a crew saw cutting the asphalt and they did not have the required PPE on. After talking with the foreman, it was discovered that the JSA had not been completed prior to work starting. I stopped all work and contacted Das-Co leadership and informed them of the observed situation. Das-Co safety personnel arrived on-site and took control of the job.
