IPUC inspector Perkins observed Das-Co a contractor for IGC dig a trench to install 3/4" service in
star, Idaho. Das-Co personnel dug the trench using an expired locate ticket 311922 (8/30/23)
To addresses: 12581 W Shortcreek St., Service line#009710-367. Observations included:
Safety equipment, trenching. All procedures were not followed. FOC John Hezeltine was present when I
discovered the locate ticket was expired.
