Inspection Summary
Audit consisted of inspecting farm taps for the Lewiston and Moscow districts, new construction, reg
station work and FOP/curb valves for Avista Utilities.
Feb 27,2023: (2.0 Standard travel/inspection)
Traveled to Lewiston, ID to conduct inspections and inspected Moscow farm taps
Feb 28,2023: (2.0 Standard)
Met with Avista company representative Trevor Salonen and accompanied him inspecting farm taps in Moscow,
Troy, Genesee, Lewiston and Deary, Idaho. Inspection consisted of checking for leaks, wrap condition at soil
interface, vegetation issues, any kind of damage and rust/pitting issues.
Farm tap#385 in Troy had a leak on the regulator and#3784 in Moscow had a leak on the regulator and relief.
Pressure controlman repaired all leaks on the spot and there were no further issues.
Mar 1,2023: (2.0 Construction)
IPUC inspectors observed Avista crew install a new 3/4" service line to 418 Linden Ave in Lewiston, Idaho.
Observation included: line locating, thumper boring, welding on service tapping tee, wrapping,pressure testing,
stab fittings, EFV install,tracer wire,bedding,backfilling,riser install and meter install/testing. All procedures
were followed in accordance with Avista GSM manual. We also observed Avista crew personnel checking
emergency operation valves and curb valves. Valve#69 was difficult to operate and took two personnel to turn,
crew personnel annotated this and valve will be cut out and replaced within 90 days.

Observed Trevor Salonen replacing pilot valve heater on regulator station#406 at Clearwater Paper as well
checking lockup on worker and monitor regs on both legs of the station.
Mar 3,2023: (2.0 Standard Travel)
Traveled back to Boise, ID.
Mar 9,2023: (0.5 Standard Post, 0.5 OQ)
