ID.
Addresses: 2059 S Grand Fork Way
Observations Included: Excavation, trench depth, safety equipment, verification of OQs, equipment
calibrations, tracer wire, stab fitting, fusions, line squeeze, EFV installation, successful pressure test (96
psi for 15 minutes), bedding and backfill. Multiple AOC's were correctly recognized involving saddle
maintenance and existing stub damage that were mitigated. All procedures were followed in accordance
with IGC Operations and Maintenance Manual.
