IPUC inspector Perkins observed Das-Co, a contractor for IGC start installing a 2" service line in Boise,
ID.
Address: 3083 E Barber Valley Dr.
Observations Include: Fusing(EFV/Riser), verification of OQ records, line locating ,scaffolding was in
the way to complete trenching, unmarked sewer line discovered, fire extinguishers were both expired for
inspection and big extinguisher had been discharged/out of green zone on dial. Cord on heater/iron had
a frayed plug-in and advised to replace. JSA was not filled out prior to commencing job. PPE gear was
not worn as required. Quizzed personnel on excavation rules/laws. All observed safety procedures were
not followed in accordance with IGC Operations and Maintenance Manual. I called IGC/Das-co
leadership and informed them of the discrepancies.
