IPUC inspector Perkins observed Crown, a contractor for IGC fuse a 2"Tee to 2"main lines in
Meridian, ID.
Address: : Pera Place subdivision on the corner of Dapne &N Colosseum ave
Observations Include: Fusing, verification of OQ records. Quizzed personnel on separation of gas lines
from other utilities and steps for fusing. All observed procedures were not followed in accordance with
IGC Operations and Maintenance Manual.
