Observations Included: Excavation, trench depth, safety equipment, verification of OQs, equipment
calibrations, tracer wire, plastic butt fusion, riser placement, EFV 1200 installations,. Quizzed
employees on applicable fusion equipment inspections and related AOCs. All procedures were
followed in accordance with IGC Operations and Maintenance Manual.

Gas service installation was not performed due to foreman recognizing an AOC involving new stub
