IPUC inspector Galli traveled to and observed KV a contractor for IGC install 2"main line(s) in Sugar
City, ID.
Address: River Edge Subdivision Sugar City, ID
Observations Include: correct trench depth, line locating, tracer.wire, safety equipment, verification of
OQ records, verification of equipment calibrations, boring and pullback under roadway, electrofusion to
4"main, 2" electrofusion coupling, 2"butt fusions, pigging, successful pressure completed in two
segments at 97 psi for 15 minutes and 96 psi for 2 hours, proper use of mechanical fittings (bolt-on T's
and couplers) and EFV installation, tap wrap on carrier pipe around sleeved areas. Quizzed employees
on applicable AOCs of fusion procedures and mechanical fittings.
Discrepancies: 2"butt fusions at a T were cut out due to uneven rollback, gouge/nick type anomaly
found in the roll back. The carrier pipe was originally found unprotected around all sleeve ends.
MDU compliance personnel Colby Lundstrom and Josh Sanders were made aware of the sleeve issues
and escalated to the Idaho Falls District Manager Charles Haszier. FOC Jake Robertson responded to
the area to investigate and advised the contractor to fix the issues.
