ID.
Address: 875 E Cleveland Ave
Observations Include: (Customer provided trench, correct trench depth), safety equipment, fusing,
verification of OQ records, tracer wire, setting of riser. Quizzed personnel on separation of gas lines
from other utilities and rough handling procedures. All observed procedures were followed in
accordance with IGC Operations and Maintenance Manual.
