Observations Included: Excavation, trench depth, safety equipment, verification of OQ, equipment
calibrations, tracer wire installation,plastic butt fusions, line squeeze(release not witnessed), EFV 1200
installations at each service, successful pressure test at 96 psi for 15 minutes, and backfill. Quizzed
employees on plastic butt fusions and procedures. All procedures were followed in accordance with
IGC Operations and Maintenance Manual.
