Address: 119 Marvin St, Caldwell, ID
Observations Include: Correct trench depth, line locating, boring, safety equipment, verification of OQ
records, verification of equipment calibrations, tracer wire, butt fusion, EFV 1200, 2-inch saddle T,
successful pressure test, bedding, backfill and continuity testing. Quizzed employee on applicable
AOCs of fusion and saddle procedures. All observed procedures were conducted in accordance with
IGCs Operations and Maintenance Manual.
