Additional Comments:
IPUC inspector Galli observed Crown Utilities a contractor for IGC install 2"main in the Oaks North
#12 subdivision in Meridian, ID.
Address: Oaks North#12 Meridian, ID
Observations Include: Correct trench depth, safety equipment, verification of OQ records, verification
of equipment calibrations, tracer wire and butt fusion. Quizzed employee on applicable AOCs of fusion
procedures.
Discrepancies and Violations:
• Observed an expired pyrometer dated 02/2024 and was replaced with an unexpired pyrometer
dated 05/2024.
• Observed contractor fuse a 2"pipe and end cap together without refacing after adjusting the
clamp. FOC Will Lynsey was notified and LOC I202403 will be written to IGC for failure to
comply with OPS 605, OPS 307 and TR-33. FOC instructed the contractor to cut out the fuse
and redo it.
