Address: 1956 Gage Ave, Twin Falls, ID Observations Include: correct trench depth, line locating, safety equipment, verification of OQ records, verification of equipment calibrations, tracer wire, EFV 1200 installation, stab fittings (couplers and T's), bedding(sand not needed) and backfill. Quizzed employee on applicable AOCs of squeeze off and bedding procedures. Contractor failed to conduct a continuity test of the tracer wire per OPS 307 Section 6.11 with a locating device. IGC FOC Jake Hunter and KV Manager Jason Gordon were notified and issue was remediated.
