Inspection Summary
Audit consisted of IPUC inspectors Perkins and Brooks inspecting field installed operating equipment for
the Avista Moscow inspection unit.
May 6,2024: (2.0 Standard Travel)
Traveled to Moscow, ID to conduct field inspection.
May 7, 2024: (2.0 Standard)
Met with Avista company representatives in the Pullman, Washington(Moscow) district office for a briefing,
reviewed welding records,past year work orders and began field audit of Moscow installed equipment.
11 regulator stations were inspected for relief operation, regulator lock-up, general condition of the station
including pipe-to-soil wrap, and cathodic protection. 14 EOP/Emerg Valves were checked for operation and ease
of turn. 1 Carrier/Casing at Howell &HWY 8 to check reading to ensure they were not grounded.

Wrote inspection report, completed audit forms: PHMSA IA question set with OQ protocol 9, IPUC Form 1,
Post-Inspection Preliminary Written Findings and attached inspection documents.Traveled back to Boise.
Conducted brief fireside meeting regarding compliance items and potential PHMSA agreement change.
