January 17,2024: (1.0 Standard Prep JB) Prepared for all Avista field audits. April 22,2024: (2.0 Standard Travel) Traveled to Lewiston, ID to conduct field inspection. Met with Heath a contractor for Avista to observe annual A/C and US work in Moscow, ID. OQs verfied, equipment calibrations verfied and contractors were quizzed on applicable AOCs in regard to found discrepancies of leaks and corrosion. April 23-24,2024: (4.0 Standard) Met with Avista company representatives in the Clarkston, Washington district office for a briefing, reviewed welding records, past year work orders and began field audit of Lewiston installed equipment. 5 regulator stations were inspected for relief operation,regulator lock-up, general condition of the station including pipe-to-soil wrap, and cathodic protection. One farm tap was inspected for adequate cathodic protection. Five rectifiers, two carrier-to-casings and sixteen valves were also inspected. Two regulators (416&4020)were found to have minor discrepances that marginally affected the regulators mechanical ability to completely lock-up allowing a slow increase in downstream pressure. -Regulator Station 416: A small amount of oil was found on the valve stem of the worker and believed to come from an upstream Williams compressor station. Additionally,the worker valve stem had a small indentation that possibly affected the stem to sit tight on the orifice. -Regulator Station 4020: A small indentation was found on the monitor valve stem that possibly affected the stem to sit tight on the orifice. Additionally,the worker valve stem was found to have a very slight amount of dithiazine that allowed the regulator to slowly increase the downstream pressure. Inspectors met with the Avista Moscow valve crew during their annual maintenance inspections and observed them operate and maintain 4 valves in Moscow, ID. OQs were verified and employees were quizzed on applicable ADCs of inoperative valves. April 25,2024: (2.5 Standard Travel/Post/Compliance) Wrote inspection report, completed audit forms: PHMSA IA question set with OQ protocol 9, IPUC Form 1, Post-Inspection Preliminary Written Findings and attached inspection documents. Traveled back to Boise. Conducted brief fireside meeting regarding compliance items and potential PHMSA agreement change.
