You are hereby notified that you are delinquent in payment for water service provided to you by
Syringa Water Company, Inc. The balance owed on your account is indicated above.
If this balance is not paid or if payment arrangements are not made before the date shown above,
then your water service will be disconnected. See IDAPA 31.21.01.305. That for purposes of
termination, partial payments will be applied toward utility service charges first, unless the
customer requests otherwise, and that charges for non-utility services cannot be used as a basis for
termination.
To avoid termination ofservice you must contact Syringa Water Inc. before the termination date
to make a payment arrangement or to pay in full. Please call on weekdays between 9AM and 6
PM.
Termination may be delayed by: Providing a medical certificate advising ofthe existence ofa
medical emergency; or filing a complaint regarding the proposed termination with the Idaho
Public Utilities Commission , Po Box 83720, Boise, Id. 83720-0074, 800-432-0369 or
208-334-0300.
Service will not be terminated prior to resolution ofa filed complaint. Termination of service
does not relieve you ofyour obligation to pay for all services prior to termination.
If you are unable to pay your bill in full, Syringa Water Company,Inc. may assist
you by making payment arrangements.
Remember,your water will be turned off on or after the termination date shown above unless you
act before that date.

NOTICE OF RECONNECTION
Due to delinquent payment for water service provided to you by Syringa Water Inc. Syringa
Water Inc. has terminated water service. To reestablish water service, the customer must take the
following steps.
• Contact Syringa Water Company, Inc. Monday through Friday between 9AM and 6 PM.
• Pay balance owed on your account as indicated above.
• If needed, make a payment arrangement to pay in full.
• Schedule a time to reconnect during regular office hours.
Remember, for water service to be reestablished, customer must adhere to Syringa Water
Company, Inc.'s notice of reconnection.
