2020 Field Inspection Dominion Energy
August 10, 2020: (1 Standard Prep)
Audit consisted of inspecting field installed equipment and operator qualification for the Franklin & Preston,
Idaho inspection units. Records and procedures for all Dominion Energy Gas Units were inspected in April 2020
via a virtual/teleconference with Dominions compliance personnel

Traveled from Boise, ID to Logan, UT.
August 18, 2018: (.5 Standard, .5 OQ)
Idaho Public Utilities Commission Inspector met with Dominion Energies compliance and engineering
personnel at Dominion's Offices in Logan, Utah for in-brief and to begin the field audit. The PHMSA
IA Form 2 (Gas Distribution Inspection) and PHMSA Form 15 (OQ Field Inspection Form) were used as
a guides throughout the field audit to ensure all areas were completed.
Regulator Stations FR0001, FC0001, FC0003, FC0004, FC0005, FC0006, and PS0001 were inspected for proper
operating parameters, regulator lock-up, relief device operation, cathodic protection pipe-to-soil readings, pipe
to soil interface wrap protection, station warning signs and right-of-way clearances.
Four casing-to-carrier isolation checks, one cathodic protection rectifier and nine emergency isolation valves
were checked for proper operation and found to be operating properly.
An IPUC Form 3 (exit interview)was performed following the inspection with Lauren Skufca (Pipeline
Compliance), Clark Jarrett (Measurement& Control (M&C)Tech), Ron Kiiha (CP Tech),James Sharp (M&C
Lead) of Dominion Energy.
Observations:
Reg Stations: FR0001, FC0001, FC0003, FC0004, FC0005, and FC0006 not plumbed for lock-up.
