2020 Drug&Alcohol/Operator Qualification Program Inspections
October 2, 2020: (1 D&A prep)
Prepared for the Dominion Energy's Drug and Alcohol/Operator Qualification program inspections through
review of the PHMSA OPS Inspector Assistant question sets and the operator's DOT Anti-Drug and
Alcohol/Operator Qualification Plans. Audits will be conducted jointly with Utah and Wyoming.

Traveled to Salt Lake City, Utah to the Dominion Energy headquarters for the audit.
October 6, 2020: (2 D&A)
Darrin Ulmer and Bruce Perkins of the Idaho Public Utilities Commission met with DEUWI's Lauren Skufca
(Engineer, Pipeline Compliance) and Elizabeth (Ann) Kirby (Senior Occupational Health Specialist (Virtually)) at
DEUWI's Office in SLC, UT to review their DOT Anti-Drug and Alcohol Plan. PHMSA's Substance Abuse Program
Comprehensive Audit and Inspection Protocol Inspector Assistant questions were used as a guide throughout
the audit to ensure all areas were completed.
October 7, 2020: (2 OQ)
Darrin Ulmer and Bruce Perkins of the Idaho Public Utilities Commission met with DEUWI's Lauren Skufca
(Engineer, Pipeline Compliance) and Cathy Mendez (Operator Qualification Program Compliance Specialist) at
DEUWI's Office in SLC, UT to review their Operator Qualification Program. PHMSA's Operator Qualification
Inspector Assistant questions were used as a guide throughout the audit to ensure all areas were completed.

Traveled back to Boise.

Typed the inspection report, completed forms: PHMSA IA questions, IPUC Forms 1 & 3 and attached inspection
documents.
