Inspection Summary
Audit consisted of inspecting field installed operating equipment and operator training for the Avista
Sandpoint,Idaho inspection unit. Procedures for all Idaho Avista units were inspected during the COVID-
19 lockdown April 2020,the records inspection was postponed until 2021.
July 24,2020: (2 Standard prep)
Prepared for the field inspection of Avista Utility Company natural gas distribution pipeline for the Sandpoint,
Idaho inspection unit. Reviewed operators Operations and Maintenance manuals, Emergency manuals, and
Operator Qualification procedures. Prepared PHMSA Form 2 and IPUC worksheets for the inspection.
July 27,2020: (2 Standard travel)
Traveled to Sandpoint, lD to conduct inspection.
July 28,2020: (2 Standard)
Met with Avista company representatives in the Sandpoint, Idaho district office for in-brief and began field audit
of Sandpoint installed equipment. David Applebaum(PHMSA)also attended the inspection while auditing the
PUC.
Regulator Stations 205, 2050, 206, 615, 616, 618 and 620 were inspected for relief operation, regulator lock-up
and general condition of the station including pipe-to-soil wrap and corrosion. Two cathodic rectifiers, six steel
emergency valves, one patrol and three carrier-to-casings were inspected for the Sandpoint district.
Observation: No deficiencies noted during inspection, recommended sign be replaced on Carrier/Casing at
Syringa and Ontario. IPUC Form 3 was conducted with Randy Bareither, Jim Kane,Jeff Lee, Steve Finney and
Shawn Slinkard.
July 29,2020: (1 Construction)
Observed an Avista crew install two 3/4" PE service lines off 1 1/4" main in Sandpoint, Idaho. Areas observed
were excavation,pipe handling/placement, tracer wire installation, welding,meter placement, EFV installation,
pressure testing,padding, and backfill.
July 30,2020: (2 Standard travel)
Traveled back to Boise, ID.
July 31,2020: (2 Standard post)
Wrote the inspection report, completed audit forms: PHMSA Form 2, IPUC Form 1,IPUC Form 4 and attached
inspection documents.
