HomeMy WebLinkAboutIntermountain Gas 20 Dec Construction Idaho PUC Pipeline Safety Inspection Summary Inspector(s): Perkins/Russo Date(s): 20-Dec-19 Company/Owner: Intermountain Gas Company Unit/OPID: 8160 Inspection Type &Time (1 day) Total Inspection Days: 2 Standard: Construction: F2 Training: (T)(D)Integrity Management: Incident: M/M: Follow-up: OQ: Damage Prevention: D&A: Inspection Summary A construction inspection was performed on Das-co, a contractor for Intermountain Gas Company. 28 Oct 2019: (2.0 Construction) Observed a Dasco crew excavating for a new 6" main and fusing the new 6" pipe. While observing the fusing crew we noticed that they were not checking the iron temperature prior to fusing as per IGC procedural requirement. Prior to the next fuse I asked them to show me what the temperature of the iron was, they got the pyrometer and I asked to check the calibration date and it was within specs. They then proceeded to check the temps and it was reading between 448-450 degrees, and the required temps are 500 + or— 10 degrees per IGC procedure. At that time, we contacted IGC leadership and informed them of what had taken place and they sent IGC personnel and Das-Co to stop the job. They disqualified the fuser, and made the site safe for the day and stopped all work.
