HomeMy WebLinkAboutIntermountain Gas 17 Jul Construction Idaho PUC Pipeline Safety Inspection Summary Inspector(s): I Perkins/Ulmer I Date(s): 17-Jul-19 Company/Owner: Intermountain Gas Company Unit/OPID: 8160 Inspection Type &Time (1 day, 0.5 day) Total Inspection Days: 2 Standard: F] Construction: 72 Training: F__] (T)(D) Integrity Management: E::] Incident: EJ M/M: F1 Follow-up: F-1 OQ: F-1 Damage Prevention: El D&A: EJ Inspection Summary A construction inspection was performed on DAS-CO, a contractor for Intermountain Gas Company. 17 July 2019: (2.0 Construction) Crew was installing new 3/4" service line and abandoning 2" Aldyl-A piping while using the 2"Aldyl-A as a sleeve for the new service line. When we arrived the crew was excavating to expose the 2" main that tied into the 2" Aldyl-A which was being abandoned. Previous to our arrival the crew had struck approximately 8 unlocated fiber/internet/cable lines and had not ceased excavation. According to Idaho Law section 55-2207, Chapter 22 states that "An excavator must hand-dig within twenty-four(24) inches of an underground facility to determine the precise location. If an excavator discovers underground facilities that are not identified or not accurately located, he is to stop excavating in that vicinity and immediately notify the established call center or facility owner." "An excavator who damages an underground facility must notify the facility owner and must not bury the facility until the facility owner has the opportunity to look at the facility and make necessary repairs." Employees continued the excavation using only shovels to expose the 2" main. Century Link personnel were called and arrived to confirm that 6 of the lines that were struck were still active lines and made the necessary repairs. After exposing the main the welder stripped the coating and welded on a 2" line stopper fitting and did a burst test/bubble test to ensure the weld was error free to accommodate the tapping/plugging tool so that the 2"Aldyl-A piping could be blown down and the 3/" new service line inserted. Welder then performed the tap/plug of the 2" main. Employees then installed new service line and pressure tested. Once this was completed the welder installed new wrap on the steel line. Typed report. Areas observed during this project were: Line locations, trenching, installing new service line and tracer wire, installing saddle T's and EFV, pressure testing, bedding/backfilling, Attachments: IPUC Form 4 Construction Inspections DAS-CO Daily Itineraries located in file for the project. OQ's for all observed employees are located in file for the project.
