Inspection Summary
Audit consisted of inspecting field installed equipment and operator qualification for the Boise, Idaho inspection
units. Records and procedures for all Idaho Intermountain Gas Units were inspected in February 2019 at the
Intermountain Gas Company (IGC) headquarters in Boise, Idaho.
April 3,2019: (2 Standard Prep)
Prepared for the field audit of Intermountain Gas Company's natural gas distribution pipeline for the Boise,
Idaho inspection unit. Reviewed operators operations/maintenance manuals, emergency manuals, operator
qualification procedures and the previous years audit results. Prepared PHMSA Form 2 and IPUC Forms and
worksheets for the inspection.
April 8-11, 2019: (9 Standard and 3 OQ)
Met with IGC company representatives in the Boise district office for in-brief and to begin field audit.
Regulator Stations 40026,40030,40032,40033,40040,40044, 40047,40048, 40049,40050,40051,40053,
40055,40056, 40058,40061, 40062, 40065,40067, 40070,40072,40078,40080, 40085,40086,40087, 40088,
40091,40092,40094,40096,40097, 40098 and 40101 were inspected for proper operating parameters,
regulator lock-up, relief device operation, cathodic protection pipe-to-soil readings, pipe-to soil interface wrap
protection,station warning signs and right-of-way clearances.
Eight casing-to-carrier isolation checks, three cathodic protection rectifier,five pipeline patrols, and ten
emergency isolation valves were checked for proper operation, observations listed below.
An operator qualification inspection was performed on Brandon Croy an IGC employee that was performing
cathodic protection system: electrical connections. Brandon performed the task in a satisfactory manner.
An operator qualification inspection was performed on Shawn Leasy an IGC employee that was performing
valve maintenance: inspection/partial operation. Shawn performed the task in a satisfactory manner.
An operator qualification inspection was performed on Chris Coe an IGC employee that was performing
pressure regulating limiting/relief device operation and maintenance. Chris performed the task in a satisfactory
An IPUC Form 3 (exit interview) was performed following the district inspection with Roger Phillips (Operations
Manager), Kelley Gorringe (Compliance/Audit Specialist), Greg Watkins (Manager,Standards &Compliance) and
Chris Coe (Meter Inspector) of Intermountain Gas Company.

Observations:
Not Plumbed for lock-up: Reg Stations: 40086, 40048,40087,40091 and 40092.
Failed lock-up: Reg Stations: 40072, 40085, 40058,40049,40032, 40098 and 40094, corrected on the spot.
OQ:Justin Koff when asked for his OQ Card couldn't produce, said it was in his filing cabinet in the office.
Low CP Read: C2C: 42509702, 42509703 Reg Station: 40048, CP Tech is investigating.
