Inspection Summary
Audit consisted of records and inspecting field installed equipment for all of Dominions operating system in
Idaho.
Aug 23, 2019: (1 Standard Prep)
Prepared for the Dominion Energy records inspection through review of the PHMSA inspections Forms 2 and
2018 inspection results. Dominion Energy's procedure standards will be inspected during next month's
procedural audit. The company's pipeline system in Idaho consists of transmission and distribution lines.

Traveled to Logan, UT to conduct a records audit as well as a field inspection

Met with company representatives in Logan office to review records
Aug 28,2019: (3 Standard)
Met with company representatives at the Logan office for in-brief and to begin field audit.
Regulator Stations and Bird Cages FC0001, FC0003, FC0004, FC0005, FC0006, FR0001 and PS0001 were
inspected for proper operating parameters, regulator lock-up, relief device operation, cathodic protection pipe-
to-soil readings, pipe-to soil interface wrap protection, station warning signs and right-of-way clearances.
4 casing-to-carrier isolation checks, 1 cathodic protection rectifier and 4 emergency isolation valves were
checked for proper operation and found to be operating properly.
An IPUC Form 3 (exit interview) was performed following the district inspection with Lauren Skufca (Pipeline
Compliance)of Dominian Energy.
Observations:
Bird Cages not plumbed for lock-up.
