Inspection Summary
Audit consisted of inspecting Dominion Energy U/W/1 procedures. This audit was performed jointly by Idaho
and Utah State pipeline safety inspectors in Salt Lake City, Utah at Dominions Energy's headquarters and
consisted of travel to and from Salt Lake City, Utah. The Procedures were audited together by all inspectors
with a separate inspection of the individual documentation for each state by their state inspector. Each State
is submitting audit reports, forms and letters as required by their individual states.

Prepared for the Emergency Plan audit of Dominion Energy. Reviewed operator's Emergency Plan, prepared
PHMSA Form 2, and IPUC Form 1.
Nov 12,2019 (2 Standard travel)
Traveled to Salt Lake City, Utah to the Dominion Energy headquarters for the Emergency Plan audit.
Nov 13, 2019: (2 Standard)
Met with Dominion Energy representatives to audit the companies Emergency Plan.
Nov 14, 2019: (2 Standard)
Met with Dominion representatives to finish up the Emergency Plan audit. Traveled to Park City, Utah to
observe a walk through of the companies Emergency response on-site command center trailer. Following the
audit an IPUC Form 3 exit interview was conducted with Lauren Skufca - Pipeline Safety Engineer, Miriam
Santos-Technical Writer and Chris Burnett -Technical Writer.

Completed PHMSA Form 2 and IPUC Form 1 for final review and filing.
