2019 Records Audit Avista Utilities
February 22, 2019: (2 Standard prep)
Prepared for the Avista records inspection through review of the PHMSA inspection Form 2 and 2018 inspection
results. Avista's procedure standards were reviewed and compared to the PHMSA Form 2 checklist for
completeness. The company's pipeline system in Idaho consists of distribution lines only. Avista has no
transmission pipelines or compressor stations in Idaho.
February 25, 2019: (3 Standard)
Traveled to Spokane, WA to conduct inspection.
February 26-28, 2019: (9 Standard)
Darrin Ulmer, Bruce Perkins and Kyle Russo of the Idaho Public Utilities Commission met with Avista's
compliance and engineering personnel at Avista's headquarters in Spokane, WA to review records and
procedures for the previous year 2018. The PHMSA Inspector Assistant (IA) question set for records was used
as a guide throughout the audit to ensure all areas were completed. A procedure review was just completed
last year and was not accomplished. The field portion of the checklist will be used throughout calendar year
2019 at Avista's field units to ensure compliance with field requirements.
The inspection noted two areas of concern. Avista couldn't demonstrate the effectiveness of their Emergency
Response Performance. IPUC reviewed an emergency response in Lewiston and the after-action report noted
numerous gaps in the response. Avista could not demonstrate that Liaisons with Public Officials are being
maintained. Public Safety Specialist came in and showed a tracking spreadsheet of all events, IPUC noted
numerous missing supporting forms as documentation that the event happened.

Traveled back to Boise, ID.
March 6, 2019: (1 Standard post)
Wrote the inspection report, completed audit forms and attached inspection documents.
