Inspection Summary
Audit consisted of inspecting field installed operating equipment and operator training for the Avista
Moscow,Idaho inspection unit. Records and procedures for all Idaho Avista units were inspected
February 26-28,2019 at the Avista headquarters in Spokane,Washington.
May 17,2019: (1 Standard prep)
Prepared for the field inspection of Avista Utility Company natural gas distribution pipeline for the Moscow,
Idaho inspection unit. Reviewed operators Operations and Maintenance manuals, Emergency manuals, and
Operator Qualification procedures. Prepared PHMSA Form 2 and IPUC worksheets for the inspection.
May 19,2019: (3 Standard travel)
Traveled to Moscow, ID to conduct inspection.
May 20-21,2019: (4.5 Standard& 1.5 Construction)
Met with Avista company representatives in the Pullman, Washington district office for in-brief and began field
audit of Moscow installed equipment.
Regulator Stations 3785, 3065, 338, 321, (324 & 367 were converted to single service farm taps) were inspected
for relief operation,regulator lock-up and general condition of the station including pipe-to-soil wrap and
corrosion. One cathodic rectifiers,ten steel emergency valves and three carrier-to-casings were inspected for the
Moscow district. IPUC Form 3 was conducted with Randy Bareither, Elizabeth Frederiksen, Jeff Lee and Trevor
Salonen.
Observations:
Reg Station 3065,was in harmonic failure, replace regulator and tested
An Avista crew was installing 900' of 2"PE from 2"Stub. Customer ditch and bedding. Stub out near planned
transformer approx. 200' down hill. Stub 3/4"at end of trend for future installs. Areas observed during this project
were: plastic pipe joining,pressure testing,tracer wire installation, and backfilling.

Traveled back to Boise, ID.
May 31,2019: (1 Standard post)
Wrote the inspection report, completed audit forms: PHMSA Form 2, IPUC Form 1 and attached inspection
documents.
