April 4,2019: (1 Standard prep) Prepared for the field Audit of Avista Utility Company natural gas distribution pipeline for the Lewiston, Idaho inspection unit. Reviewed operators Operations and Maintenance manuals, Emergency manuals, and Operator Qualification procedures. Prepared PHMSA Form 2 and IPUC worksheets for the inspection. April 15,2019: (1 Standard travel) Traveled to Lewiston, ID to conduct inspection. April 16-17,2019: (1 Standard, .5 OQ, .5 Construction) Met with Avista company representatives in the Clarkson,Washington district office for in-brief and began field audit of Lewiston installed equipment. Regulator Stations 401, 402, 404, 406, 410, 415 and 416 were inspected and randomly selected for relief operation, regulator lock-up and general condition of the station including pipe-to-soil wrap and corrosion. Five cathodic rectifiers, nine emergency and one secondary valves and three carrier-to-casings were inspected for the Lewiston district with no defects noted. IPUC Form 3 was conducted with Randy Bareither, Jeff Lee and Trevor Salonen. An operator qualification inspection was performed on Trevor Salonen an Avista employee that was performing Regulator Station Annual Maintenance. Trevor performed the task in a satisfactory manner. Observed an Avista company crew cut out a service tee and valve from a 4" steel main on 21 st St in Lewiston, Idaho. Areas observed were welding, wrapping, purging, abandonment and the use of safety equipment. April 18,2019: (1 Training) Inspector observed the training and evaluation for operator welding qualification. One welder(Jake Neilson) was tested and evaluated on Procedure B4 Weld Category: Production,Non-Pressurized, Manual Shielded Metal Arc (SMAW). Observed the following destructive testing on welders coupons: Tensile Test,Bend Test and Nick-Break Test. April 29,2019: (1 Standard post) Wrote the inspection report, completed audit forms: PHMSA Form 2, IPUC Form 1 and attached inspection documents.
