Records and procedures for all Idaho Avista units were inspected March 26-28,2019 at the Avista headquarters in Spokane,Washington. May 24,2019: (1 Standard prep) Prepared for the field Audit of Avista Utilities natural gas distribution pipeline for the CDA and Kellogg, Idaho inspection units. Reviewed operators Operations and Maintenance manuals, Emergency manuals, and Operator Qualification procedures. Prepared PHMSA Form 2 and IPUC worksheets for the inspection. June 3,2019: (3 Standard travel) Traveled to CDA, ID to conduct inspection. June 4-5,2019: (4.5 Standard, 1.5 OQ) Met with Avista company representatives in the CDA and Kellogg, Idaho district offices for an in-brief and began field audits of CDA and Kellogg installed equipment. Regulator Stations 216, 217, 220, 222, 228, 240, 248, 252, 253, 260 622, 623 and 2210 were inspected and randomly selected for relief operation, regulator lock-up and general condition of the station including pipe-to- soil wrap and corrosion. Three cathodic rectifiers,ten emergency valves, eight carrier-to-casings,three patrols and farm taps 1359, 2677, 3648, 4859, 6074 and 8683 were inspected for the CDA/Kellogg districts with no defects noted. IPUC Form 3 was conducted with Randy Bareither, Jesse Butler, David McGregor, Steve Finney, Jake Nielsen and Jeff Lee. An operator qualification inspection was performed on John "Jake"Nielsen an Avista employee that was performing Valve Maintenance Certification. Jake performed the task in a satisfactory manner. June 6,2019: (3 Standard travel) Traveled back to Boise, ID. June 7, 2019: (2 Standard post) Wrote the inspection report, completed audit forms: PHMSA Form 2, IPUC Form 1 and attached inspection documents.
