Records and procedures for all Idaho Intermountain Gas Units were inspected in March 2018 at the Intermountain Gas Company(IGC) headquarters in Boise, Idaho. August 20, 2018: (1 Standard Prep) Prepared for the field audit of Intermountain Gas Company's natural gas distribution pipeline for the Nampa, Idaho inspection unit. Reviewed operators operations/maintenance manuals, emergency manuals, operator qualification procedures and the previous years audit results. Prepared PHMSA Form 2 and IPUC Forms and worksheets for the inspection. 21 Aug,2018: (2 Standard) Met with IGC company representatives in the Nampa district office for in-brief and to begin field audit. Ten casing-to-carrier isolation checks, one cathodic protection rectifier, one pipeline patrol, and thirteen emergency isolation valves were checked for proper operation and found to be operating properly. 22-23 Aug, 2018: (22 Aug-2 Standard, 23 Aug-4 Standard) Regulator Stations 30016, 30018, 30046, 30047, 30051, 30056, 30062, 30069, 30070, 30074,40106, 40107, 40110, 40114, 40115,40116,40119,40120,40121,40125,40126,40127, 40128, 40177, and 40188 were inspected for proper operating parameters, regulator lock-up, relief device operation, cathodic protection pipe- to-soil readings, pipe-to soil interface wrap protection, station warning signs and right-of-way clearances. An IPUC Form 3 (exit interview) was performed following the district inspection with Tyler Muzzana (District Manager), Kelley Gorringe (Pipeline Safety Specialist), and Dave Call (Meter Inspector) of Intermountain Gas Company. Observations: Reg Station: 40120 not plumbed for lock-up. Reg Stations: 30056, 30074 and 40116 (Secondary)failed to lock-up, meter inspector corrected on the spot. Reg Stations: 30118,40106,40119 and 40114 (Primary failed to lock-up, meter inspector corrected on the spot. Reg Station: 30069, (Primary)failed to lock-up, bare steel on both primary/secondary risers above wrap, exposed control wiring to pressure sensor/monitor(broken conduit). Reg station: 40127, bare steel on both primary/secondary risers above wrap. Reg station: 40119, relief flapper was bent (not sitting flush). IPUC Form 1 Idaho PUC Pipeline Safety Inspection Summary Reg Station: 40107, no carsonites on north side on road crossings. Check for carsonites within class locations and all public road crossings. Odorizer: 30070 unable to get a CP read. Casings: 26701306, 26701305 and 26701309,vents are too close to the ground and are susceptible to water intrusion. Carrier: 26701306, unable to get a carrier read due to possible broken lead (per CP Tech). Casing: 26701315,vent is buried. August 24, 2018: (3 Standard Post) Completed forms and typed report.
