2018 Records, D&A, and Field Inspection Dominion Energy
September 14, 2018: (1 Standard Prep)
Prepared for the Dominion Energy records inspection through review of the PHMSA inspections Forms 2 and
2017 inspection results. Dominion Energy's procedure standards will be inspected during next month's
procedural audit that will be conducted jointly with the state of Utah. The company's pipeline system in Idaho
consists of transmission and distribution lines.
September 17, 2018: (3 Standard Travel)
Traveled from Boise, ID to Logan, UT.
September 18, 2018: (1.5 Standard, 1.5 D&A)
Idaho Public Utilities Commission Inspectors met with Dominion Energies compliance and engineering
personnel at Dominion's Offices in Logan, Utah to review records and Drug and Alcohol for the
previous year 2017. The PHMSA Form 2 (Standard Inspection Report of a Gas Distribution Operator)
and PHMSA Form 13 (D&A Inspection Form) were used as a guides throughout the audit to ensure all
areas were completed as they pertain to required records and procedures.
September 19, 2018: (3 Standard)
Met with Dominion Energy representatives in the Logan district office for in-brief and to begin field audit.
Regulator Stations FR0001, FC0001, FC0003, FC0004, FC0005, FC0006, and PS0001 were inspected for proper
operating parameters, regulator lock-up, relief device operation, cathodic protection pipe-to-soil readings, pipe
to soil interface wrap protection, station warning signs and right-of-way clearances.
Four casing-to-carrier isolation checks, one cathodic protection rectifier and nine emergency isolation valves
were checked for proper operation and found to be operating properly.
An IPUC Form 3 (exit interview) was performed following the inspection with Lauren Skufca (Pipeline
Compliance), Clark Jarrett (Measurement& Control (M&C)Tech), Jeff Hinds (CP Tech),Todd Rhoads(M&C
Tech) and Mike Thayne (M&C Tech) of Dominion Energy.
Observations:
Reg Station: FC0001 Monitor regulator failed to lock-up.
Reg Stations: FR0001, FC0001, FC0003, FC0004, FC0005, and FC0006 not plumbed for lock-up.
