Inspection Summary
Audit consisted of inspecting Dominion Energy U/W/I procedures. This audit was performed jointly by Idaho
and Utah State pipeline safety inspectors in Salt Lake City, Utah at Dominions Energy's headquarters and
consisted of travel to and from Salt Lake City, Utah. The Procedures were audited together by all inspectors
with a separate inspection of the individual documentation for each state by their state inspector. Each State
is submitting audit reports,forms and letters as required by their individual states.

Prepared for the Procedural Audit of Dominion Energy. Reviewed operator's Standard Practices and
Emergency Plan, prepared PHMSA Form 2, and IPUC Form 1.
Oct 8, 2018 (1 Standard travel)
Traveled to Salt Lake City, Utah for the Dominion Energy headquarters Procedural audit.
Oct 9-10, 2018: (2 Standard)
Met with Dominion Energy representatives to Audit the companies procedures in their Standard Practices.
Following the audit an IPUC Form 3 exit interview was conducted with Lauren Skufca - Pipeline Safety Engineer,
Miriam Santos -Technical Writer and Chris Burnett -Technical Writer.
Observations/Recommendations:
EP Sec 3 (6.1.3) lists halt machine as first item to be used, operator stated it's not used as frequently.
Suggestion that it be moved down on the list. SP 2-10-01 (6.1) Utah inspector observed modified clamps being
used and welders only completing 25%of the root pass before removing alignment clamps. SP 3-15-01 (9)
What are the NDT limits for acceptable vs. unacceptable for porosity, cracks, slag etc. SP 3-15-01 (7.1) need
example of NDT procedure.

Completed PHMSA Form 2 and IPUC Form 1 for final review and filing.
