2018 Records Audit Avista Utilities
February 20,2018: (1 Standard prep)
Prepared for the Avista records inspection through review of the PHMSA inspection Form 2 and 2017 inspection
results. Avista's procedure standards were reviewed and compared to the PHMSA Form 2 checklist for
completeness. The company's pipeline system in Idaho consists of distribution lines only. Avista has no
transmission pipelines or compressor stations in Idaho.
March 5, 2018: (3 Standard)
Traveled to Spokane, WA to conduct inspection.
March 6-7, 2018: (4.5 Standard, 1.5 D&A)
Darrin Ulmer, Bruce Perkins and Kyle Russo of the Idaho Public Utilities Commission met with Avista's
compliance and engineering personnel at Avista's headquarters in Spokane, WA to review records and
procedures for the previous year 2017. The PHMSA Form 2 (Standard Inspection Report of a Gas Distribution
Operator) was used as a guide throughout the audit to ensure all areas were completed as they pertain to
required records and procedures. The field portion of the checklist will be used throughout calendar year 2018
at Avista's field units to ensure compliance with field requirements. PHMSA Form 13 was used for the Drug and
Alcohol Audit.
As a result of the inspection one area of concern was noted. Avista had one emergency valve that fell just
outside the requirement of 49 CFR 192.747 Valve Maintenance: Distribution systems- ... must be checked and
serviced at intervals not exceeding 15 months, but at least once each calendar year. Valve LEC 709 was
installed on Jan 8, 2016 and inspected on Apr 26, 2017. The inspection was approximately two weeks past the

The crew was installing a new 3/4" plastic service line (new conversion) to the residence of 15219 N. Stevens St.
in Rathdrum, ID. The second job was not complete due to concern for the residents, refer to IPUC Form 4.
Areas observed during this project were: gas line locating, excavation, plastic service line installation (tracer
wire), plastic pipe joining, hot tapping, mechanical couplings, service tee w/EFV installation, pressure testing,
purging, riser and meter set location, bedding and backfilling.
An Operator Qualification inspection was performed on Jerome Moss the Avista Gas Apprentice on a crew in
Coeur d'Alene District while performing Pressure Testing Gas Pipelines. Jerome was fully aware of AOC's and
performed the task in a satisfactory manner.
