Inspection Summary
Construction inspections were performed on Avista crews in the Coeur d'Alene area.
February 5, 2018: (3 Standard)
Traveled to Spokane, WA to conduct inspection.
February 6, 2018: (3 Construction)
The crew was bedding and backfilling a 6" plastic main that was trenched and joined the day before near the
intersection of Chilco Rd and Penobscot Rd near Athol, ID.
Areas observed during this project were: locating other utilities, excavation, main installation (tracer wire),
bedding and backfilling.
February 7, 2018: (3 Operator Training)
Met with Avista personnel at their Jack Stewart Training Center and received a tour of the complex. Sat in and
observed a Self-Contained Breathing Apparatus (SCBA) refresher class where employees and contractors had to
don the gear and pinch off a simulated gas leak on PE and steel pipe. Fire extinguisher training consisted of
simulated scenarios of three pipe fires(one stub, one service line and one service line in a trench). Observed an
Avista trainer conduct actual Operator Qualifications on: Mechanical Spigot and Sleeve type fitting and PE Pipe
Joining on: Mechanical Couplings, Mechanical Service Tees, Electrofusion and Hydraulic Butt Fusion.
February 8, 2018: (3 Construction)
The crew was installing 2" plastic main and 3/4" plastic service lines leaving stubs for a new housing
development in the Cedar Creek Subdivision south of 1500 S Greensferry Rd, CDA, ID.
Areas observed during this project were:gas line locating, excavation, plastic main installation (tracer wire),
plastic service line installation (tracer wire), plastic pipe joining, hot tapping, electrofusion, mechanical
couplings, service tee w/EFV installation, pressure testing, purging, stub locating, bedding and backfilling.
An Operator Qualification inspection was performed on C.J. Leonard the Avista Gas Apprentice on a crew in
Coeur d'Alene District while performing Pipeline cover, clearance and backfilling. C.J was fully aware of AOC's
and performed the task in a satisfactory manner.
February 9, 2018: (3 Standard)
Traveled back to Boise, ID.
