Inspection Summary
Audit consisted of inspecting Field installed operating equipment and Operator Qualification for the Avista
Coeur d' Alene/Kellogg, Idaho inspection units. Records and procedures for all Idaho Avista units were
inspected in March 5-9, 2018 at the Avista headquarters in Spokane, Washington.
June 1, 2018 (1 Standard prep)
Prepared for the field Audit of Avista Utility Company natural gas distribution pipeline for the Coeur d'
Alene/Kellogg, Idaho inspection units. Reviewed operators Operations and Maintenance manuals, Emergency
manuals, and Operator Qualification procedures. Prepared PHMSA Form 2, and PUC worksheets for the
inspection.
June 4, 2018 (2 Standard travel)
Traveled to Coeur d' Alene, Idaho for Avista field review.
June 5, 2018 (1 Standard & 1 OQ)
Met with Avista Company representatives in the Kellogg, Idaho district office for in-brief and begin field audit of
Kellogg, Idaho area installed equipment. Kellogg's location specific emergency plan was verified and reviewed
for accuracy.
Regulator Stations 232, 235-1, 235-2, 244, 242, 249, 243, 270, 275, 273 and 279 were inspected for proper
operating parameters, cathodic protection pipe-to-soil readings, pipe-to-soil interface wrap protection, station
warning signs and right-of-way clearances.
Two carrier-to-casing isolation checks, three cathodic protection rectifiers, one patrol crossing and two emergency
isolation valves were checked for proper operation.
An Operator Qualification inspection was performed on Steve Finney the pressure controlman for Coeur d'Alene
District while performing regulator station annual maintenance. Steve was fully aware of AOC's and performed
the task in a satisfactory manner.
An out brief(IPUC Form 3, exit interview) was performed following the district inspection with the district
manager and Randy Bareither, Compliance Manager for Avista Utilities.
June 6, 2018 (2 Standard)
Met with Avista company representatives in the Coeur d'Alene, Idaho district office for in-brief and begin field
audit of Coeur d'Alene area installed equipment. Coeur d'Alene location specific emergency plan was verified
and reviewed for accuracy and unit welding qualification documents were checked and found error free.
Regulator Stations 210, 212, 214, 215-1, 215-2, 221, 621 and 2210 were inspected for proper operating parameters,
cathodic protection pipe-to-soil readings, pipe-to-soil interface wrap protection, station warning signs and right-of-
way clearances.

June 7, 2018: (2 Construction)
The crew was installing a new 6" main westward along 21 st Ave between Idaho St & Spokane St in Post Falls,
Idaho (#28036180004)
Areas observed during this project were: Other utility location, Material handling, Bedding, Excavation, Steel pipe
joining, Jeeping, Pipe installation, Tracer wire installation, OQ, and Equipment calibration

Pipe was buried at a depth of 60".
Crew was safety conscience throughout the project.
June 8,2018 (2 Standard travel)
Traveled back to IPUC office.
June 13,2017: (1 Post)
Wrote the inspection report for the Coeur d'Alene/Kellogg, Idaho area field review of Avista installed operating
equipment. Completed PHMSA checklist and PUC Excel sheets.
