Dear Mrs. Morehouse:
On June 7-8, 2022, the Idaho Public Utilities Commission, Pipeline Safety Division, pursuant to
Chapter 601 of Title 49,United States Code, conducted the field inspection of Avista's installed
equipment in the Coeur d' Alene District.
There are area(s) of concern that do not fully meet the intent of the pipeline safety regulations
Title 49, Code of Federal Regulations, Part 192 for calendar year 2022.
The item(s) listed below are of concern:
49 CFR�192.739 (a) Pressure limiting and regulating stations: Inspection and testing.
(2) Adequate from the standpoint of capacity and reliability of operation for the service in
which it is employed;
(3) Except as provided in paragraph (b) of this section, set to control or relieve at the correct
pressure consistent with the pressure limits of§ 192.201(a); and
(4) Properly installed and protected from dirt, liquids, or other conditions that might
prevent proper operation.

Regulator stations 212, 4246, and 4247 had one or both regulators fail to lock up at the pre-
established set points. These failures were corrected on-site and at the time of failure.

The above mentioned item(s) were brought to the attention of your compliance representatives
during the inspection and exit interview. We would request that you review these matter(s)
and respond in writing within 30 days regarding the above issue including any planned
corrective actions.
