Price 2626 Eldorado Boise ID 83704 Subject: Sanitary Survey conducted on March 27, 2019 Dear Robert H. Price: On March 27, 2019, we conducted a Sanitary Survey for Capitol Water Corp. I am enclosing a list of findings for your system. Please consult with the Department of Environmental Quality (DEQ)within 30 days regarding any significant deficiencies identified in this written notification, as required by IDAPA 58.01.08. After consultation and agreement between the public water system (PWS) and DEQ, please provide the regulating agency a written corrective action plan addressing all significant deficiencies. All new public water systems (PWSs), or modifications to existing PWSs, must be submitted to DEQ in a preliminary engineering report and approved by DEQ prior to the submittal of plans and specifications, as required by IDAPA 58.01.08.503.01. Following approval of the preliminary engineering report, plans and specifications must be submitted to and approved by DEQ prior to construction of new PWSs or modifications of existing PWSs, as required by IDAPA 58.01.08.504. In order to address all significant deficiencies, follow steps 1, 2, 3 & 4. Step 1: Within 30 days of receiving this written notification, submit to me a"Planned Completion Date" for each significant deficiency item. Step 2: Complete the planned action(s) by the "Planned Completion Date(s)." Step 3: After completing each planned action, enter an "Actual Completion Date," your initials, and write the "Corrective action taken." Step 4: Sign your name at the bottom certifying that each corrective action has been corrected by the planned completion date and that your PWS has completed the Sanitary Survey response requirements pursuant to IDAPA 58.01.08. Send me a copy of the signed paperwork. Significant Deficiencies Please check all wellhouses to make sure that all sounding tubes are capped, and install caps on any that are uncapped. Planned Completion Date: Actual Completion Date. Initials Corrective action taken: Please check all wellhouses to ensure that vents are equipped with 24 mesh screens. Planned Completion Date: Actual Completion Date: Initials Corrective action taken: Please confirm that the oil used in the well pumps is NSF approved, and if it is not, replace with oil that is. Planned Completion Date: Actual Completion Date: Initials Corrective action taken: Please install a spill containment structure around the auxiliary power generator. A concrete curb large enough to hold 110% of the fuel tank volume is an acceptable solution. Signature: Date: Deficiencies Please decide the future of Well #2 and abandon it per Idaho Code if there are no plans to use it. Recommendations As a reminder, any time the distribution system drops below 20 psi, the PWS must provide public notice and disinfect the system. DEQ recommends that a leak detection program be put in place and utilized. DEQ recommends all systems utilize the SMART management financial tool which is now available for use on the Public Drinking Water System Switchboard under the heading "Tools/Data." The financial tool is designed to gather information about your system's revenues, expenses, assets, liabilities, debts, and reserves to assist with assessing your overall financial health.
