THIS IS AN IMPORTANT NOTICE REGARDING CHANGES TO YOUR
CENTURYLINK VOICE SERVICES
We’re sending this notification to let you know that we are making changes that affect your CenturyLink
voice services effective May 15, 2020.
If you do not agree to the changes to tariffs, catalogs, schedules, and other terms and conditions, then
you must take the steps discussed below by May 14, 2020.
Based on applicable tariffs, catalogs, schedules and other terms and conditions, we want to remind you
that you’re billed in advance for recurring monthly charges associated with your voice services, including
charges for features including but not limited to caller ID and voicemail (“Voice Services”), and the
applicable taxes and fees for these items.
What will change?
As of May 15, 2020, or as soon after that date as authorized by state regulatory commissions, your Voice
Services and applicable taxes and fees will not be prorated if you cancel these services on any day other
than the last day of your billing cycle. These services will continue to be available to you until the end of
your billing cycle and you will not receive a refund (pro-rated credits) for any days remaining in your
billing cycle after you cancel Voice Services.
If you do not agree to be bound by the updated tariffs, catalogs, and rate schedules, please call
CenturyLink at 1-866-554-6777 by May 14, 2020 to discuss options, which may include cancellation of
your Voice Services. If you instead continue to use your Voice Services, that will constitute your
acceptance of the updated tariffs, catalogs, and rate schedules.

Minimum charges for business service applies when the entire service is discontinued within one calendar month of the service establishment date. The minimum charge will consist of the monthly business rate. When business service is terminated at the customer’s request after the minimum service period or initial contract period has been met, service will be discontinued immediately, and a final bill will be issued in accordance with Section 2.3.2.A.5. Residential customers may request termination of residence service at any time, and service will be discontinued on the last day of the customer’s billing cycle after the minimum service period has been met. Final charges will be rendered in accordance with Section 2.3.2.A.5. 1. Service and equipment charges do not apply to disconnect and/or removal of service, equipment, service features or facilities unless otherwise specified. 2. After the expiration of the initial contract period, service may be terminated upon reasonable advance notice to the Company and payment of all charges due to the date of termination of the service, except that, in the case of joint user services, each directory period will be considered as a separate initial contract period, (i.e., listings will be automatically included in each directory unless sufficient advance notice to do otherwise is received from the listed party) and termination may be arranged for only under the conditions specified in 3.b., following. Prior to the expiration of the initial contract period, service may be terminated upon reasonable advance notice to the Company and upon payment of the termination charges hereinafter provided, in addition to all charges for the period service has been rendered. a. In case the initial contract period is one month or three months, the termination charge consists of the charges for the remainder of the initial contract period. b. Directory additional listings and joint user services (1) Where the listing appears in the current directory, charges to the end of the directory period will apply except that the charges will cease at the time: • The contract for the main service is terminated. • The listed party or joint user becomes a subscriber to some class of exchange service. • The listed party or joint user dies or moves to some new location at which the customer's service in connection with which he is listed is not available. GENERAL REGULATIONS - CONDITIONS OF OFFERING 2.3 PAYMENT FOR SERVICE 2.3.1 CUSTOMER RESPONSIBILITY The customer is responsible for payment of all charges for facilities and services furnished the customer, including charges for services originated, or charges accepted, at such facilities. 2.3.2 PAYMENT OF BILLS A. Prorating of Opening and Closing Bills Opening and closing bills for business services will be prorated on the basis of a thirty-day month. Exceptions are services with a specific minimum billing period. On message rate service, the message allowance for a fraction of a month will also be prorated. Bills rendered for establishment of residence services will be prorated on the basis of a thirty-day month. Final bills rendered after customer-requested discontinuance of residential service will not be pro-rated and service will remain available to the customer until the first day of the customer’s next billing cycle. B. Payment of Bills 1. All charges for exchange service, equipment, and facilities exclusive of charges for local messages in excess of the monthly allowance and toll message, are payable monthly in advance. Charges for local messages in excess of the monthly allowance and toll messages are payable monthly except that the Company reserves the right to require payment of such charges at more frequent intervals. 2. Where the rates and charges for a particular service are determined by applying a percentage or similar factor to a quoted rate or charge and such computation results in a fraction, the charge for the service shall be computed to the nearest cent, a half cent being increased to the next higher cent. 3. In the event a customer is indebted to the Company for charges and services previously rendered, or for service under one or more numbers at the same location, and the customer does not pay the charges or satisfy such indebtedness, the Company may charge and bill such indebtedness for a business account against the customer's business service. In the event that payment from a customer is less than the total amount of all charges owing to the Company and the customer does not specifically designate the manner in which he wishes to apply said payment, then the Company will apply the payment to each entity based on the ratio of the entity balance to the total account balance. 5. Payment of bills for telephone service may be made by any means mutually acceptable to the customer and the company. Payment which is not honored or paid by the customer’s designated financial institution will be considered as nonpayment. 6. Customers have the following options as to the method of paying bills for telephone service: • At any Company payment depository location. • At the office of any authorized payment agent of the Company. • By U.S. Mail, by check or money order only. • Through an agent of the customer. • By electronic funds transfer. 7. Payments received by the Company on or before the due and payable date on the customer's bill will be considered timely, provide the following billing information is remitted with payment: • Customer's name. • Customer's telephone number. • Customer's customer code. • Customer's account type. • Amount of payment. 8. Payments received by the Company after the due and payable date on the customer's bill, but at least one day before the termination date on the suspension notice, may result in discontinuance of the customer's service unless the following billing information is remitted with payment: • All of the items enumerated in 7., and • The final payment date before discontinuance for nonpayment. These Plans are not available with the following: • Information Delivery Service sponsor charges • Other Company Discount Calling Plans • Other Company MTS Discount Plans • Toll Only Accounts 2. When the business customer initially subscribes to or disconnects this service, if a partial month is billed, the customer's minutes and monthly rate will be prorated. When a residence customer initially subscribes to this service, the customer’s minutes and monthly rate will be prorated. Final bills for disconnection of residence service will be rendered in accordance with Section 2.3.2.A. 3. These Plans are available on an account level basis, where one or more lines are billed to the same account. 4. Where the customer has one account which includes multiple lines, these Plans apply to total usage of combined lines. 5. These Plans apply to all intraLATA dial station-to-station, calling card, operator-assisted and pay telephone originated calls (where the call is billed to the customer's telephone number) unless otherwise specified. 6. These Plans are available (where billing capabilities exist) to all business, residence and PAL accounts with the restrictions and limitations set forth elsewhere. 7. The rates and charges for this service are in addition to the rates and charges for the local exchange service with which it is associated. 8. The minimum service period is one month. 9. Nonrecurring charges do not apply.
