Dear ldaho Public Utilities Commission:
Pursuant to IDAPA 31.01.01 .114, Ziply Fiber Pacific, LLC doing business as Ziply Fiber submits
the following information for consideration to be authorized as a competitive localexchange
carrier in the state of ldaho.
1. A short statement of the character of public servlce in which the entity is engaged.
Ziply Fiber Pacific, LLC is a new entrant in Idaho telecommunications. Ziply Fiber
Pacific is a telephone corporation within the definition of ldaho Statutes 61-121and 62-
603 and a facilities based competitor under 62-203. lt is currently registered as a
Competitive Local Exchange Carrier in Oregon and Washington. lt is afiiliated with Ziply
Fiber Northwest, LLC and Ziply Fiber of ldaho, LLC, !LEC's which provide public
telecommunications services in the State of ldaho, both voice and data, to residents and
business.
2. The entity's name (including any assumed business name) and the state in which it is
incorporated or organized.

3. The street and mailing addresses of the entity's principal office and of its principal office
in ldaho, and emailaddress (if available);

7. !f not incorporated or organized in ldaho, a certificate of authority from the ldaho
Secretary of State, a certificate of good standing issued by the secretary of sate in the
state the entity is incorporated or organized, and the name and street address of the
enti$'s registered agent for service in ldaho; and the name and address of any
corporation, association, or similar organization holding a five percent (5%) or greater
ownership interest or a managerial interest in it, and the amount and nature of the
ownership interest, and nature of the management interest. lnclude a copy of any
management agreement with the application.
Ziply Fiber Pacific, LLC is a wholly owned subsidiary of Northwest Fiber, LLC. Attached
please find our certificate of authority issued by the ldaho Secretary of State and our
certificate of good standing issued by the Delaware Secretary of State. There are no
management agreements between companies.
8. Adescription of customer classes and customer seruices that the applicant proposes to
offer to the public. The application shall indicate the date on which the applicant
proposes to begin construction or anticipates it wlll begin to provide service in ldaho.
We plan to serve residences and businesses with fiber-based telecommunications
services, including internet access, data transport and voice over IP services. We
anticipate construction to begin in early 2023.

9. A description suffcient to determine whether service is to be offered in a particular
location and the names of incumbent localexchange corporations (!LECs) with whom
the proposed utility is likely to compete. The application shall also describe the intended
manner of service, e.9., resold services or facilities-based services; and a general
description of the property owned or controlled by applicant.
Preliminarily,Ziply Fiber Pacific plans to provide services over its own facilities in the
following areas: Boise, ldaho Falls, Lewiston, Meridian, Nampa, Pocatello, Rexburg and
Twin Falls. Ziply Fiber Pacific may expand its services to other areas in the future. The
ILEC in the areas cunenUy planned for service are CenturyLink.
10. A reasonably sized and detailed map showing where the applicant proposes to provide
service including exchanges (if different fiom existing exchanges), rural zones, and local
calling areas. lf the service area is identicalto an incumbent LEC's service area, then
applicant may refer to the incumbent's service area.

11. The current detailed balance sheets, including detailed income and profit and loss
statements of applicant reflecting current and prior year balance for the twelve (12)
months ending as of the date of the balance sheet, or if not readily available, for the
period since the close of the preceding calendar year. lf a balance sheet and income
statement are not available, the applicant shall submit financial data sufiicient to
establish it possesses adequate financial resources to provide the proposed services.
Ziply Fiber Pacific, as a new company, does not have balance sheets to provide. ln lieu
of furnishing specific balance sheets, attached are the balance sheets of Northwest
Fiber, Ziply Fiber Pacific's parent company.
12.The latest annual report, if any.
Ziply Fiber Pacific does not produce an annual report.
13. Tariffs and Price Lists. Proposed initialtarifis or price sheets setting forth rates, rules,
terms, and regulations applicable to the contemplated service. lnitialtariffs and price lists
filings shall be in an electronic form as well as paper. The tariffs and price lists in
electronic format will be in computer searchable Adobe Acrobat (PDF), or submitted on a
CD'ROM or other format as prescribed by the Commission Secretary.
We plan to offer services that are not subject to tariff in the State of ldaho. These
services are lP-based services, including internet access, data transport and voice-over-
lP. Our terms and conditions of service will be published on our web site and will be
substantially similar to the attached.
14. Tariff and Customer Contact. The name, address, and telephone number for those
persons responsible for tarifi and price list questions, as well as customer complaints and

inquiries. The application shall state the toll-free telephone numberfor customer inquiries
and complaints.

15. lnterconnection Agreements. \Mether the applicant has initiated interconnection
negotiations and, if so, when and with whom. lnclude copies of any interconnection
contracts which have been completed for the provision of telecommunication services.

16. Compliance with Commission Rules. Awritten statement that the applicant has
reviewed the Commission's rules and will comply, or request for waiver of those rules
believed to be inapplicable, or both.
Ziply Fiber Pacific, LLC has reviewed and will comply with the Commission's rules or will
request a waiver where appropriate.
17. Conservation of Telephone Numbers. An acknowledgment that non-paging
telecommunications carriers with telephone numbering resources in ldaho shall be
subject to numbering conservation measures including mandatory one thousand (1,000)
block pooling. See Commission Order No. 3O425. AIICLECS shallevaluate their
numbering resources and donate to the numbering resource pool unused one thousand
(1,000) number blocks and one thousand (1,000) number blocks that have fewer than
ten percent (10%) of the telephone numbers assigned. Applicable caniers shall also ftle
the necessary utilization reports wlth NeuStar and semi-annual report their number
resource utilization/forecast (NRUF) data at the one thousand (1,000) block level for
each rate center within their service tenitory. The Federal Communications Commission
has appointed NeuStar to manage the assignment and conservation of telephone area
codes and telephone numbers in North America.
Ziply Fiber Pacific, LLC acknowledges that non-paging telecommunications caniers with
telephone numbering resources in ldaho shall be subject to numbering conservation
measures including mandatory one thousand (1,000) block pooling.
