(“Company”)1. This Certificate authorizes the Company to provide wholesale transport, retail
broadband, Voice over Internet Protocol (“VoIP”) service, and local exchange services in Idaho.
The Commission approved the Company’s request for a Certificate by Order No. 35618 issued
December 8, 2022 and authorized the Company to provide telecommunications services in the
State of Idaho. This Certificate is predicated upon and is issued pursuant to the findings of the
Order and is effective for service performed on or after December 8, 2022.
