Wide Voice proposes to provide resold and facilities-based local exchange services through the combination of its own facilities and the resale of other carriers' facilities and network elements. The Company intends to offer service immediately upon certification and approval of its interconnection agreement. Wide Voice will be providing service to business customers. Service Territory Wide Voice intends to offer service in the entire state of Idaho. Financial Information Wide Voice possesses the managerial, technical and financial ability to provide local telecommunications service in the State of Idaho. Wide Voice has the financial resources to enable the Company to successfully provide local telecommunications service in the State of Idaho and the management team in place to manage its operations. Wide Voice provides its financial statements as proof of its financial stability to provide the required services within the State of Idaho as Exhibit E. A copy of current resumes of Wide Voice's management personnel is attached as Exhibit F. InterconnectionAgreements The Company is in the process of negotiating interconnection agreements with Qwest Corporation dba CenturyLink QC and will have these agreements filed with the Commission prior to offering any local services. Escrow Account or Security Bond (If a company required advance deposits by its customers, the company must submit a signed copy ofan escrow account with a bonded escrow agent or a security bond. The escrow or bond shall be sufficient to meet customer deposit refunds in case of company default.) (At the commission discretion, an additional deposit may be required to keep customers whole in case of company default) (The Commission will review the individual requirement of establishing an escrow or security account by the Company upon good showing by the Company for a period of two years. Wide Voice will not be requiring any advanced payments or prepaid services that will require a bond. Mr. Chicas was a co-founder of the CLEC and VoIP provider CommPartners, LLC in June 2003 to 2008, serving as its Chief Technology Officer. Mr. Chicas is responsible for the network architecture and topology, vendor selection, POP construction and all engineering aspects of the Company. Mr. Nickerson has over 13 years' experience in the telecommunications industry, spending the previous 4 years with leading Skype & Microsoft's carrier partnerships and business development initiatives. In addition, he was the CFO of a VoIP service provider startup that raised over $l8M in venture funding and served customers like AT&T, Sprint, Vodafone and Telefonica. Wide Voice Management Biographies (Continued) Tandy DeCosta, (Co-founder) - Director, Telephony Services Ms. DeCosta is a co-founder of Wide Voice, LLC and serves as Director of Telephony Services. Ms DeCosta handles all aspects of day-to-day operations. Ms. DeCosta is responsible for all day-to-day operations of Wide Voice, LLC, including regulatory, CLEC build out, traffic engineering and network planning. From 2004 to 2010 Ms DeCosta was Director of Translations at CommPartners, LLC handling LCR routing, traffic engineering, SS7 and network engineering and build out. From 1999 to 2004 Ms. DeCosta worked at Qwest Wireless as a Tier 5 support in their Advanced Technical Assistance Center, as well as handling routing/translations in DMS and 5ESS switches. From l99l to 1999 Ms. DeCosta worked at GTE Mobilnet (now Verizon), in Hawaii, handling RF engineering as well as cell site management and troubleshooting. From 1986 to l99l Ms. DeCosta worked for Honolulu Cellular (now AT&T wireless), handling cell site management, including install and RF engineering. Erla Erlingsdottir, Senior Manager of Telephony Semices Ms. Erlingsdottir has recently joined Wide Voice,LLC to help manage states. CLEC build out to 50 Ms. Erlingsdottir is responsible for managing and implementing all facets of the 50 state CLEC build out for Wide Voice, LLC. Prior to joining the company Ms. Erlingsdottir worked at Level3 Communications from January 2009 to August 20 13 as a Product Manager as well as a Senior Software Engineer, handling all routing and costing functions, including updating production environments. From October 2004 to January 2009 Ms. Erlingsdottir worked at CommPartners handling routing, LNP, SS7, trunk builds and other switch build out provisioning. From April 2001 to October 2004 Ms. Erlingsdottir worked at Qwest Wireless, as a Wireless Translations Engineer, handling routing, technical support and product design for the CO region, until Qwest Wireless decided to outsource their network to Sprint. From October 1999 to April 2001 Ms. Erlingsdottir worked at ICG Communications as a Complex Translations Engineer, handling switch routing and trunk builds, as well as provisioning process improvements.
