Pursuant to Title 62 of the Idaho Code and IDAPA 3l .01.1 I l, Wide Voice, LLC ("Wide Voice")
respectfully requests that the Idaho Public Utilities Commission ("Commission") to grant the Company a
Certificate of Public Convenience and Necessity to provide local exchange telecommunications services
within the State of Idaho.
In support of its Application, Wide Voice submits the following:
I. Proposed Seruices:
Wide Voice, LLC, proposes to offer competitive local exchange service, including exchange
access service, within the state of Idaho using its own facilities. It may also utilize resold services
available from the underlying ILEC or other facilities-based carriers. Petitioner will negotiate an
interconnection agreement with Qwest Corporation dba CenturyLink QC.

III.Telecommunication Service
The Applicant anticipates it will begin to provide service as soon as this application is approved
by the Commission. Wide Voice proposes to provide resold and facilities-based local exchange
services through the combination of its own facilities and the resale of other carriers' facilities and
network elements. The Company intends to offer service immediately upon certification and
approval of its interconnection agreement. Wide Voice will be providing service to business
customers.
Service Territory
Wide Voice intends to offer service in the entire state of Idaho.
Financial Information
Wide Voice possesses the managerial, technical and financial ability to provide local
telecommunications service in the State of Idaho. Wide Voice has the financial resources to
enable the Company to successfully provide local telecommunications service in the State of
Idaho and the management team in place to manage its operations. Wide Voice provides its
financial statements as proof of its financial stability to provide the required services within the
State of Idaho as Exhibit E. A copy of current resumes of Wide Voice's management personnel
is attached as Exhibit F.

The Applicant's proposed local and access tariffs are included with this application as Exhibit H.

Applicant is responsible for all customer inquiries and complaints. The telephone number for
customer inquiries and complaints is provided by Wide Voice on the customer bill. Wide Voice's

V[I. InterconnectionAgreements
The Company is in the process of negotiating interconnection agreements with Qwest
Corporation dba CenturyLink QC and will have these agreements filed with the Commission
prior to offering any local services.
IX. Compliance with Commission Rules
The Applicant has reviewed all of the Commission rules and agrees to comply with them.

X. Escrow Account or Security Bond
(If a company required advance deposits by its customers, the company must submit a signed
copy ofan escrow account with a bonded escrow agent or a security bond. The escrow or bond
shall be sufficient to meet customer deposit refunds in case of company default.)
(At the commission discretion, an additional deposit may be required to keep customers whole in
case of company default)
(The Commission will review the individual requirement of establishing an escrow or security
account by the Company upon good showing by the Company for a period of two years.
Wide Voice will not be requiring any advanced payments or prepaid services that will require a
bond.

Patrick Chicas, President & Chief Technical Oficer
Mr. Chicas is the founder of WideVoice, LLC and serves as its President and Chief Technical Officer.
Mr. Chicas was a co-founder of the CLEC and VoIP provider CommPartners, LLC in June 2003 to 2008,
serving as its Chief Technology Officer.
Mr. Chicas is responsible for the network architecture and topology, vendor selection, POP construction
and all engineering aspects of the Company. From August 2000 to November 2003, Mr. Chicas was the
president, co-Chairman and a member of the board at Rubicon Media Group, an lnternet publishing
concern. From March 1999 to August 2000, Mr. Chicas was the Vice President for Data Services at
Mpower Communications. While at Mpower, Mr. Chicas designed the company's VoIP network for
small business services. From January 1997 to September 1998, Mr. Chicas was the first executive hire
and Vice President of Operations at Digital Island, lnc. Mr. Chicas has prior telecommunications
experience with PacTel Cellular (now Verizon) and GTE Mobilnet (now Verizon). As an early participant
in Internet-related businesses, Mr. Chicas was a co-founder of Hawaii Online's Intemet Access Service.
Hawaii Online was the first statewide ISP in Hawaii and was ultimately sold to GST Communications.
Mr. Chicas also cofounded LJ.net, an ISP founded to serve rural communities in the western United
States. Mr. Chicas resides in Las Vegas, Nevada.
Andrew Nickerson, Chief Executive Officer
Mr. Nickerson joined Wide Voice, LLC as CEO in August 2014 to continue its successful growth of
connecting applications and services to the PSTN. Mr. Nickerson has over 13 years' experience in the
telecommunications industry, spending the previous 4 years with leading Skype & Microsoft's carrier
partnerships and business development initiatives. Prior to Skype, Mr. Nickerson s served as an
independent consultant to several application providers, financial institutions and carriers interested in
expanding their business in the global communications industry. In addition, he was the CFO of a VoIP
service provider startup that raised over $l8M in venture funding and served customers like AT&T,
Sprint, Vodafone and Telefonica.
Mr. Nickerson earned a Bachelor's degree in Accounting from Santa Clara University and his MBA from
The Wharton School of Business with majors in finance and entrepreneurial management. Mr. Nickerson
resided in Manhattan Beach, CA with his wife and 2 kids and in his spare time he enjoys training for and
competing in triathlons.

Tandy DeCosta, (Co-founder) - Director, Telephony Services
Ms. DeCosta is a co-founder of Wide Voice, LLC and serves as Director of Telephony Services. Ms
DeCosta handles all aspects of day-to-day operations.
Ms. DeCosta is responsible for all day-to-day operations of Wide Voice, LLC, including regulatory,
CLEC build out, traffic engineering and network planning. From 2004 to 2010 Ms DeCosta was Director
of Translations at CommPartners, LLC handling LCR routing, traffic engineering, SS7 and network
engineering and build out. From 1999 to 2004 Ms. DeCosta worked at Qwest Wireless as a Tier 5
support in their Advanced Technical Assistance Center, as well as handling routing/translations in DMS
and 5ESS switches. From l99l to 1999 Ms. DeCosta worked at GTE Mobilnet (now Verizon), in
Hawaii, handling RF engineering as well as cell site management and troubleshooting. From 1986 to
l99l Ms. DeCosta worked for Honolulu Cellular (now AT&T wireless), handling cell site management,
including install and RF engineering. From 1984-1986 Ms. DeCosta worked for Rochester Telephone
Hawaii, working on installation and translations of large PBX systems, including automatic call
distribution (ACD) and class five features.
Ms. DeCosta is originally from Hawaii but now resides in Grand Junction CO.
Erla Erlingsdottir, Senior Manager of Telephony Semices
Ms. Erlingsdottir has recently joined Wide Voice,LLC to help manage
states.
CLEC build out to 50
Ms. Erlingsdottir is responsible for managing and implementing all facets of the 50 state CLEC build out
for Wide Voice, LLC. Prior to joining the company Ms. Erlingsdottir worked at Level3 Communications
from January 2009 to August 20 13 as a Product Manager as well as a Senior Software Engineer, handling
all routing and costing functions, including updating production environments. From October 2004 to
January 2009 Ms. Erlingsdottir worked at CommPartners handling routing, LNP, SS7, trunk builds and
other switch build out provisioning. From April 2001 to October 2004 Ms. Erlingsdottir worked at Qwest
Wireless, as a Wireless Translations Engineer, handling routing, technical support and product design for
the CO region, until Qwest Wireless decided to outsource their network to Sprint. From October 1999 to
April 2001 Ms. Erlingsdottir worked at ICG Communications as a Complex Translations Engineer,
handling switch routing and trunk builds, as well as provisioning process improvements. From March
1998 to October 1999 Ms. Erlingsdottir worked at TCG/AT&T Local Services, learning customer service,
order fulfillment and finally becoming a translator in their DMS and 5ESS switches. Also handled
training other teams on provisioning processes.
Ms. Erlingsdottir resides in Denver, CO but is originally from Iceland.
